Patients Know Best is a secure online personal health record and patient portal designed to bring information from different care providers into one patient-controlled account. People can view available results, medications, appointments, correspondence and care plans, add their own information or readings, and use configured messaging and questionnaire features. Patients can grant and withdraw access for professionals, carers or family members, supporting shared care across organisational boundaries. The service may also be reached through the NHS App where enabled. The exact information and functions visible depend on local integrations and configuration, and the portal complements rather than replaces the clinical systems used by care teams.
