Central London Community Healthcare NHS Trust provides more than 105 clinical services to over four million people across 14 London boroughs and Hertfordshire. Its staff work in local clinics, community facilities and patients' homes, with the aim of delivering effective care closer to home.

The trust supports people throughout life, from health visiting and children's services to community nursing, rehabilitation, long-term condition management and palliative care. Its teams help patients remain independent, manage their health and avoid unnecessary hospital admissions or prolonged stays. CLCH also coordinates continuing healthcare across parts of West and North London and works with health, social care and voluntary partners to develop preventive, connected neighbourhood services and reduce health inequalities.
