Derbyshire Community Health Services NHS Foundation Trust is one of England's largest specialist community health providers. Around 4,400 staff serve more than 1.1 million people across Derby and Derbyshire from community hospitals, health centres, GP practices, schools, care homes and patients' homes.
Its services include community nursing and therapies, urgent treatment centres, health visiting, school nursing, community hospital beds, older people's mental health care and four community general practices. Remote consultations complement face-to-face care where clinically appropriate. By helping people recover, manage conditions and remain independent outside acute hospitals, the trust plays a central role in Joined Up Care Derbyshire, working with health, social care and voluntary partners across the county.
