Leeds Community Healthcare NHS Trust was formed in 2011 to provide community health services for the people of Leeds. Around 3,000 staff work from health centres, neighbourhood hubs, schools, hospital clinics and patients' homes, with some highly specialised services extending across Yorkshire and the Humber.

Its teams support adults, children and families through community nursing, rehabilitation, therapies, health visiting, school nursing and specialist clinical services. Neighbourhood teams help people with complex or long-term needs remain independent, while recovery hubs and early-discharge services provide an alternative to prolonged hospital care. The Trust frequently works alongside general practice, social care, charities and the criminal justice system, reflecting the practical and social factors that shape health outside hospital.
