Leeds Community Healthcare NHS Trust was formed in 2011 to provide community health services for the people of Leeds. Around 3,000 staff work from health centres, neighbourhood hubs, schools, hospital clinics and patients' homes, with some highly specialised services extending across Yorkshire and the Humber.
