Lincolnshire Community Health Services NHS Trust is the county's principal provider of community healthcare. Its services are designed to help people manage their health at home, prevent conditions from worsening and reduce avoidable admissions to acute hospitals across Lincolnshire's large rural and coastal area.

The Trust combines community hospitals and urgent care services with teams delivering community nursing, therapies, rehabilitation, reablement, palliative care, public health and children's healthcare. Staff work in local facilities and patients' homes, coordinating support with general practice, hospitals, social care and voluntary organisations. This broad out-of-hospital role allows patients to receive treatment, recover after illness and maintain independence closer to the places where they live.
