About Graphnet Remote Monitoring
Graphnet Remote Monitoring is a digital platform for managing people outside hospital, including virtual wards and longer-term condition pathways. Patients or carers submit observations, symptoms and device readings from home; clinicians review real-time information through dashboards, trend views and structured worklists. Configurable thresholds and triage rules generate alerts when results require attention, helping teams prioritise intervention rather than checking every submission equally. Graphnet supports devices used for conditions such as heart failure, COPD, diabetes and frailty, plus secure messaging, photographs, audio and video. Integration with Graphnet shared care records and some GP systems can reduce duplication, although devices, pathways, escalation rules and integrations are locally configured.
