About Nervecentre
Nervecentre supports virtual wards by combining a live patient list and operational dashboard with its EPR and Patientcentre application. Teams can track people receiving acute care at home, record assessments and observations on mobile devices, and share current information with authorised colleagues. Patients can submit physiological readings, medicines taken and clinical-form responses, with the data contributing to the same continuous care record. Automated surveys, messages and tasks can support scheduled review and escalation. The platform helps coordinate the pathway but does not define the clinical service: eligibility, monitoring devices, measurement frequency, alert thresholds, staffing and escalation arrangements must be designed and governed by the organisation operating the virtual ward.
