Bespoke My Medical Record

My Medical Record is a personal health record developed and operated by University Hospital Southampton. Available through a website and mobile app, it lets UHS patients view outpatient appointments, documents, letters and selected results while adding information such as weight, blood pressure, notes and updates about their condition. More than 50 clinical teams have developed pathway-specific features for preparation, treatment and supported self-management, including tools that can reduce unnecessary hospital visits. Patients and clinicians jointly maintain parts of the record, and users can contact relevant support services through the platform. Registration is available to people who have been referred to, tested or treated by UHS, including those who attended its emergency or eye casualty services.
