Bespoke My Health and Wellbeing

My Health and Wellbeing is the patient portal used by Wye Valley NHS Trust and developed for patients across Herefordshire and Worcestershire. People aged 16 and over can use it on a phone, tablet or computer to review outpatient appointments, receive appointment and clinical letters, find service contact details and access health information. It includes trackers for measures such as sleep, mood, pain and weight, plus a library of more than 1,000 patient-information videos. Users can create a circle of care, giving chosen family members, friends or carers permission to view information. The portal supports NHS login for patients registered with an English GP and can surface appointment information through the NHS App.
