HomeLink Healthcare Hospital at Home is a clinically led service that helps NHS partners deliver treatment in a patient's own home. It combines in-person care from nursing and therapy teams with monitoring and reporting technology that shares timely information with the referring service. HomeLink says it has provided hospital-at-home care since 2016, supporting admission avoidance and earlier discharge while operating every day of the year. This is therefore more than a standalone remote-monitoring application: clinical staffing, home visits, care coordination and technology form one service model.
