PPM+ supports virtual ward working at Leeds by making the hospital record available to the clinicians coordinating care beyond a conventional ward. Leeds has also delivered remote-monitoring virtual wards with specialist partners such as Inhealthcare, so PPM+ should not be confused with the monitoring devices or patient-facing data-collection service: its distinctive role is sharing clinical context and supporting coordinated workflow around the pathway.
