Norfolk Community Health and Care NHS Trust

Norfolk Community Health and Care NHS Trust provides more than 70 community-based services across Norfolk. Its teams work in community hospitals, clinics, GP surgeries and people's homes, focusing on care that helps patients avoid acute admission, recover after illness and remain independent within a largely rural county.

Services include community nursing, inpatient and home-based rehabilitation, physiotherapy, occupational therapy, specialist nursing, palliative care, children's services and virtual wards. Community hospitals and therapy units provide step-up and step-down support where patients need more care than can be delivered safely at home. Since April 2025 the Trust has worked in a formal group with Cambridgeshire Community Services NHS Trust under shared leadership, while retaining its statutory identity and Norfolk service focus.
