Norfolk Community Health and Care NHS Trust provides more than 70 community-based services across Norfolk. Its teams work in community hospitals, clinics, GP surgeries and people's homes, focusing on care that helps patients avoid acute admission, recover after illness and remain independent within a largely rural county.
